Making a claim

Report the accident to the police. If you don’t the TAC (Transport
Accident Commission) may not accept your claim.

Lodge a TAC claim – it is in three sections: the green claim form,
the red medical form and the blue form for loss of earnings.

The Green Form – must be lodged within a year of the accident and include
the circumstances of the claim and a signed statutory declaration.
•
The Red Form – is a medical certificate that must be completed by your
doctor.
•
The Blue Form – outlines your loss of earnings if your injury keeps
you from work.
What can be claimed?
Medical expenses, therapy, counselling and home help can all be claimed.
Loss of earnings can be claimed for up to 18 months, if you are off work. And if you are left with a permanent disability that affects your earning capacity, you may be entitled to additional compensation.
Finally, you may be entitled to a lump sum if the injury is very serious.
